Job summary

Circle Health Group in Guildford is seeking a Senior Clinical Pharmacist to join the Pharmacy team in Oncology & Haematology. The role focuses on expert clinical input, medicines optimisation, and leadership within a multidisciplinary team at Mount Alvernia Hospital.

You will lead on implementing medicines optimisation strategies, support prescribing decisions, and contribute to guidelines and policies while monitoring safety and outcomes.

Qualifications

• GPhC registration and hospital clinical pharmacy experience.
• Proven leadership and training skills in multidisciplinary teams.
• Experience with medicines optimisation and formulary guidance.

Responsibilities

• Provide expert clinical pharmacy input within MDT; conduct medicines reconciliation, clinical screening, and therapeutic drug monitoring.
• Lead medicines optimisation aligning with guidelines and cost-effectiveness.
• Support prescribing decisions via clinical assessment and formulary guidance.
• Develop and review clinical guidelines, protocols, and policies.
• Monitor medicines usage, safety, and incident reporting; participate in QA activities.

We have an opportunity for a Senior Clinical Pharmacist to join their team of staff in the Pharmacy department.

This is a full-time role for 37.5 hours a week. The role holder will be required to cover a flexible shift of this role, including:

• Provide expert clinical pharmacy input as part of the multidisciplinary team, conducting medicines reconciliation, clinical screening, and therapeutic drug monitoring.
• Optimise medicines use through evidence-based interventions, ensuring the safe and effective management of long-term and acute conditions.
• Support prescribing decisions through clinical assessment, risk evaluation, and formulary guidance.
• Lead on implementation of medicines optimisation strategies, ensuring prescribing is cost-effective, safe, and aligned with national/local guidelines.
• Contribute to the development and review of clinical guidelines, protocols, and policies.
• Monitor and report on medicines usage, compliance, and safety incidents, participating in root cause analysis and quality improvement.

Applicants should meet the following criteria:

• GPhC-registered Pharmacist.
• Significant post-registration experience in clinical pharmacy, preferably in a hospital setting.
• Strong clinical knowledge and experience in medicines communication, leadership, and multidisciplinary collaboration skills.
• Experience in mentoring or supervising junior staff.
• Must have BOPA Passport and Clinical Pharmacy Diploma.
