Job summary

The Blackheath Hospital in London is seeking a Clinical Pharmacist for a 12 Months Fixed Term role. The post requires oncology pharmacy experience and a commitment to safe, cost-effective medicines management within a multidisciplinary team.

The role covers inpatient and outpatient reviews, medication reconciliation, and support for prescribing, with a 37.5-hour week and flexible shifts. Applicants should hold an MPharm/ BSc/ BPharm and GPhC registration, with at least two years

Responsibilities

• Conduct medication reviews for inpatients/outpatients and optimise medicines.
• Provide tailored patient-facing advice on treatment plans and side effects.
• Support safe prescribing through reconciliation, screening and dose adjustments.
• Review and authorise prescriptions per guidelines and formulary.
• Promote safe, cost-effective medicine use following policies.
• Collaborate with procurement, stock control and pharmacy staff.
• Monitor medication incidents and MHRA-guided reporting.

Clinical Pharmacist - 12 Months Fixed Term

Salary: Up to £52,000.00 per year (depending on level of experience, training and qualification)

Overview

The Blackheath Hospital in London is part of Circle Health Group, Britain's leading provider of independent healthcare with a nationwide network of hospitals & clinics, performing more complex surgery than any other private healthcare provider in the country.

We have an opportunity for a Clinical Pharmacist to join their team of staff in the Pharmacy Department. Oncology experience is essential for this role.

This is a full time role for 37.5 hours a week. The role holder will be required to cover a flexible shift pattern.

Duties Of This Role Include

• Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
• Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
• Support safe prescribing through reconciliation, clinical screening, and dose adjustment.
• Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
• Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.
• Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity
• Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance

Applicants Should Meet The Following Criteria

• A degree in pharmacy MPharm, BSc or BPharm.
• GPhC registration as a Pharmacist.
• Oncology Pharmacy experience.
• Previous experience in a clinical hospital pharmacy role.
• At least 2 years of post qualification experience
• Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
• Excellent communication and clinical decision-making skills.
• Ability to work autonomously and as part of a multidisciplinary team.
• Confident using electronic prescribing and medication administration systems (e.g., EPMA).
