Responsibilities

• Provide expert clinical support and leadership to community nursing staff in Windsor & Maidenhead.
• Minimum 60% of the day spent in direct clinical patient-facing activity.
• Work flexibly over 7 days to meet the needs of the One Community Nursing service.
• Collaborate with the Operational Team Lead to ensure staff competency and ongoing clinical development.
• Champion evidence-based practice to ensure high-quality, safe patient care.
• Act as a senior clinical resource for complex patient cases.
• Promote initiatives to reduce inappropriate hospital admissions and facilitate timely discharges.
• Ensure care delivery is based on sound clinical assessment and robust care planning.
• Monitor and review staff practice to maintain high standards of care.
• Investigate complaints, safeguarding concerns and serious incidents; identify root causes and improvements.
• Add areas of concern to the risk register and disseminate learning across the team.
