Job summary

• Knowledge of Care Act 2014 and eligibility criteria.

• Knowledge of hospital discharge pathways.

Responsibilities

• Completing assessments and reviews of individuals’ care and support needs in hospitals and care settings.
• Providing support to maximise wellbeing and independence using a strengths-based approach.
• Collaborating with families, providers and health professionals to identify community resources.
• Escalating cases to senior colleagues when required.
