Job summary

Hampshire County Council in Basingstoke is seeking a Senior Case Worker to join our Hospital Teams, delivering assessments and coordinating care to support safe hospital discharges.

You will work face-to-face with patients, families and providers, using a strengths-based approach to maximise wellbeing and independence, with opportunities to collaborate and learn within a supportive environment.

Qualifications

• Strong communication, negotiation and relationship management skills.
• Ability to work with people in difficult circumstances with compassion and collaboration.
• Strong analytical and problem-solving skills.
• Sound IT and record-keeping skills.
• Ability to work independently as well as part of a team.
• Knowledge of the Care Act 2014 and its eligibility criteria.
• Knowledge of the Mental Capacity Act 2005 and Best Interest decision making
• Knowledge of hospital discharge pathways

Responsibilities

• Completing assessments and reviews of individuals’ care and support needs in hospital, local community hospitals and care homes.
• Providing support and guidance to individuals to maximise their wellbeing and independence using a strengths-based approach.
• Collaborating with individuals, their families, providers, and other healthcare professionals to find and signpost to innovative, alternative community resources to meet individuals’ needs.
• Having the confidence and knowledge to make well-informed decisions where appropriate, and escalate to senior colleagues when needed.
