Job summary

Hampshire County Council is seeking a Senior Case Worker in its Continuing Healthcare (CHC) team in Winchester. You will work with social care colleagues and other professionals to manage a diverse CHC caseload across adults with complex needs, including mental health, physical and learning disabilities.

You will support discharge-to-assess processes, participate in CHC assessments, and coordinate transitions to suitable care settings with families and care providers.

Qualifications

• Strong knowledge of the Care Act 2014 and its eligibility criteria.
• Understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (July 2022).
• Excellent verbal and written communication skills.
• Ability to work under pressure in a busy environment.
• Strong analytical and problem-solving skills.
• Good organisational and time management abilities.

Summary

As a Senior Case Worker in the Hampshire County Council Continuing Healthcare (CHC) team, you’ll work within a multidisciplinary environment collaboratively with your social care colleagues and other professionals in the varied and complex field of NHS CHC. You’ll be supported to manage a diverse caseload, working across all care groups including older and younger adults, with mental health concerns and physical and learning disabilities.

• Supporting individuals with their onward care journey following a Discharge to Assess period from hospital.
• Undertaking assessments in line with the Care Act 2014.
• Engaging with multidisciplinary teams to support completion of NHS CHC assessments.
• Collaborating with individuals and their families to transition them to suitable care provision following assessment.
