Job summary

Hampshire County Council in Guildford is seeking a Senior Case Worker to join the Continuing Healthcare (CHC) team. You’ll work in a multidisciplinary environment with social care colleagues and other professionals to manage a diverse caseload across older and younger adults with mental health, physical and learning disabilities.

You will assess under the Care Act 2014, support onward care journeys after discharge to assess, engage with teams to complete NHS CHC assessments, and help families

Qualifications

Knowledge of Care Act 2014 and its eligibility criteria.
Understanding of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care July 2022.
Good verbal and written communication skills.
Ability to work under pressure in a busy environment.
Strong analytical and problem-solving skills.
Organisational and time management skills.

Responsibilities

Supporting individuals with their onward care journey following a Discharge to Assess period from hospital.
Undertaking assessments in line with the Care Act 2014.
Engaging with multidisciplinary teams to support completion of NHS CHC assessments.
Collaborating with individuals and their families to transition them to suitable care provision following assessment.
