Job summary

Active Care Group in London is seeking an Associate Case Manager to join AJ Case Management, supporting children and adults with brain injuries and other complex conditions. The role includes client assessments, goal setting with families, and coordinating care across multidisciplinary teams.

You will travel regularly across Oxfordshire, London and Hampshire, with remote and flexible working patterns. Requirements include nursing, social work, occupational therapy, or physiotherapy, and BABICM

Qualifications

• Experience of brain injury case management
• BABICM Registered
• Knowledge of support planning, risk assessment and management plans
• Strong communication and organisational skills
• Nursing/social work/therapy qualifications preferred

Responsibilities

• Providing Case Management services to clients, children and/or adults with complex disabilities.
• Undertaking assessments of client needs and setting goals with clients and families.
• Provide regular updated reports to relevant parties within AJ Case Management standards.
• Ensure the clients’ well-being and safety is protected at all times.
• Support the client with appointments and specialist equipment, housing needs and care requirements.
• Assist with accessible holiday plans and costs.
• Maintain a mixed and varied caseload across regions.
