Job summary

Brownbill Case Management, part of the Active Care Group, seeks Associate Case Managers to support specialist case management services across London & Home Counties, Midlands and South East regions. You will advocate for clients, coordinate care packages, and manage a multi-disciplinary team to deliver tailored rehabilitation plans.

We welcome applicants with HCPC/NMC or Social Worker registration, a full driving licence, and at least 3 years in case management.

Qualifications

Minimum 3 years in a case management role.
Full UK professional health qualification (HCPC, Nursing Council, or Social Worker).
Full driving licence and access to a car.
Experience supporting people with neurological conditions in community settings.
Strong knowledge of CQC fundamental standards and KLOE.
Knowledge of MCA/DoLS and safeguarding frameworks.
Ability to address and resolve employee issues fairly.
Good organisational, time management and multi-site planning skills.

Responsibilities

Advocate for clients' needs, building strong relationships with families and solicitors.
Produce Initial Needs Assessments, action plans and SMART goals with timelines.
Recruit, select and manage a team of care/support workers.
Implement an individualised Support & Rehabilitation Plan with clear guidelines.
Coordinate MDT professionals to deliver the rehabilitation plan.
Monitor and review the Case Management Plan and Rehabilitation Plan.
Prepare risk assessments and identify risk management strategies.
