Job summary

Lower Gornal Medical Practice seeks an experienced Advanced Practitioner (ANP/ACP) to assess, diagnose and manage minor illnesses in 15-minute appointments for patients aged 2 and over. The role includes triage, long-term condition reviews, and autonomous practice within scope, with independent prescribing in line with guidelines.

The post offers a fixed slot pattern, up to three days per week, within a Monday–Friday service running 08:00–18:30.

Qualifications

• Significant post-registration experience in primary care, urgent care, or out-of-hours settings.
• Independent assessment, diagnosis and management of undifferentiated patients.
• Experience of children aged 2+ and independent prescribing in clinical settings.

Responsibilities

• Assess, diagnose and treat patients aged 2+ with minor illness in 15-minute slots.
• Triage by telephone or face-to-face and provide outreach or remote support as needed.
• Prescribe as an independent prescriber within formulary and safety standards.
• Maintain accurate clinical records (EMIS) and participate in audits and service improvement.
