Somerset NHS Foundation Trust is seeking a Practitioner for Hospital at Home Frailty Ward. The role requires registered practitioners with advanced clinical skills to operate within an ACP/trainee ACP framework, delivering autonomous assessments and treatment plans for patients at home.

The post supports flexible 8am–8pm shifts, weekend cover, and cross-cover across Somerset. Training and supervision are provided to support your development within a multidisciplinary team.

Qualificações

• Must be a registered practitioner with advanced clinical skills.
• Essential: non-medical prescribing.
• Full-time role with flexible shifts covering 8am–8pm; weekend work required.

Responsabilidades

• Attend MDT and huddles to discuss patient caseload and plan interventions.
• Carry out autonomous assessment and treatment plans for patients within the caseload.
• Prioritise time and support the team to ensure caseload is managed.
• Liaise with neighbourhood teams to enable Hospital at Home care.
