Job summary

Manchester University NHS Foundation Trust (MFT) is seeking an Advanced Clinical Practitioner to join the Manchester Community Response Teams. The post is Band 8a, permanent, full-time, and covers extended hours across seven days to support urgent community services.

The ACP will perform advanced assessments, management, prescribing, and care planning to prevent hospital admissions and enable safe home-based care within a multidisciplinary team.

Qualifications

• Advanced clinical practitioner with autonomous working style.
• Experience in acute disease management and crisis care.
• Leadership and team working across multidisciplinary settings.

Responsibilities

• Provide advanced clinical assessment and decision-making in patients in home and community settings.
• Lead and coordinate urgent community response and virtual ward pathways.
• Develop personalised care plans and ensure safe escalation and follow-up.
