Referrals may be received from NWAS, primary care, Emergency Departments, acute hospital teams and other community services. The post holder will work flexibly across South Manchester and Trafford, contributing to Urgent Community Response and Virtual Ward/Hospital at Home pathways and helping shape the continued development of an integrated Community Urgent Care Hub.

The post holder will support patients to proactively manage and anticipate future health needs by developing personalised care plans in collaboration with GPs, medical staff, Allied Health Professionals, nursing and social care teams.
The ACP will promote patient independence, self-management and proactive management of long-term and acute health needs, ensuring appropriate escalation and follow-up where required.
