Qualifications

• Experience in comprehensive geriatric assessment and frailty services.
• Ability to work effectively within a multidisciplinary team.
• Strong commitment to patient-centred care and service development.

Responsabilités

• Conduct consultant-led ward rounds and multidisciplinary board rounds.
• Assess new referrals to the Acute Frailty Hub and FSDEC.
• Review older adults in the Emergency Department with the OPAL team.
• Manage inpatients with complex frailty needs and contribute to discharge planning.
• Participate in outpatient and educational activities and service development.
