Job summary

DN Colleges Group in Scunthorpe is seeking an Inclusive Transition Coordinator to support students with SEND as they transition into college, coordinating personalised plans and liaising with schools, families and external agencies.

The role focuses on inclusion, independence and ensuring learners have opportunities to progress to adulthood, including employment and independent living.

Qualifications

• Experience of supporting young people with SEND within an education, training or related environment.
• Knowledge of SEND and Education, Health and Care Plans (EHCPs), with the ability to develop person-centred support plans.
• Excellent communication, organisational and relationship-building skills, with the ability to manage a varied caseload.

Responsibilities

• Work collaboratively with schools and education providers to support prospective learners with SEND as they transition into college.
• Assess individual learner needs and develop personalised, person-centred transition plans.
• Manage a caseload of learners, maintaining regular contact and monitoring progress against agreed actions.
• Contribute to Education, Health and Care Plan (EHCP) consultations, reviews and transition planning activities.
• Liaise effectively with learners, parents/carers, curriculum teams, support services and external agencies.
• Coordinate transition activities, including visits, familiarisation sessions and information-sharing meetings.

Education

Level 2/3 qualification in learning support, SEND, education, youth work or relevant equivalent experience
