Job summary

Doncaster College is seeking an Inclusive Transition Coordinator to support students with SEND as they transition into college. You will work with schools, families and external agencies to identify needs and coordinate support from the outset.

The role focuses on promoting inclusion, independence and positive progression, with responsibilities spanning case management, EHCP involvement and collaboration across curriculum and support services.

Qualifications

• Level 2 or Level 3 qualification in learning support, SEND, education, youth work or relevant equivalent experience.
• Level 2 qualifications in English and Mathematics (or equivalent).
• Experience of supporting young people with SEND within an education, training or related environment.
• Knowledge of SEND and EHCPs, ability to develop person-centred support plans.

Responsibilities

• Work collaboratively with schools and education providers to support prospective learners with SEND as they transition into college.
• Assess learner needs and develop personalised, person-centred transition plans.
• Manage a caseload of learners, maintaining regular contact and monitoring progress.
• Contribute to EHCP consultations, reviews and transition planning activities.
• Liaise with learners, parents/carers, curriculum teams and external agencies.
• Coordinate transition activities, including visits and information-sharing meetings.
