Job summary

Civic Recruitment Limited is seeking a SEND Progression Mentor to work across the Southwark area, delivering person-centred, outcome-focused interventions for people with learning disabilities, autism and related needs. The role covers community settings, the Southwark Resource Centre, and other sites in the London Borough of Southwark.

Qualifications

• Experience supporting individuals with Learning Disabilities, Autism, or similar needs.
• Understanding of positive behaviour support and ability to respond to behaviours of concern.
• Excellent communication and interpersonal skills with families, carers and professionals.

Responsibilities

• Deliver person-centred, strength-based and outcome-focused interventions.
• Provide tailored support to develop communication, participation and independence.
• Act as a key worker, building relationships with individuals, families and carers.
• Plan and deliver interventions that maximise social inclusion and wellbeing.
• Support access to local resources, education, training, employment and social opportunities.
• Maintain records, monitor progress and contribute to reviews and planning.
