Job summary

Civic Recruitment Limited in London is seeking a SEND Progression Mentor to join the Gateway Team on a 3 month contract, delivering person-centred, outcome-focused support to individuals with learning disabilities and related needs in the community and at the Southwark Resource Centre.

You will work flexibly to plan strength-based interventions, build supportive networks, and promote independence and social inclusion, following EHCP goals and positive behaviour support principles.

Qualifications

  • Experience supporting individuals with Learning Disabilities, Autism, Sensory Needs, Physical Disabilities, or complex additional needs.
  • A person-centred, strengths-based approach to supporting individuals to achieve positive outcomes.
  • Understanding of positive behaviour support and the ability to respond appropriately to behaviours of concern.
  • Excellent communication and interpersonal skills, with the ability to build effective relationships with individuals, families, carers, and professionals.
  • Ability to plan, organise, and deliver tailored interventions that promote independence and social inclusion.

Responsibilities

  • Deliver person-centred, strength-based and outcome-focused interventions that support individuals in achieving the outcomes identified within their EHCP.
  • Provide tailored support that develops communication skills, Encourages participation in meaningful activities, and promotes greater independence.
  • Act as a key worker, building positive relationships with individuals, their families, carers, and wider support networks to identify and achieve personalised goals.
  • Plan and deliver interventions that maximise social inclusion, wellbeing, resilience, and independent living skills.
  • Support individuals to access local community resources, voluntary sector organisations, education, training, employment, and social opportunities where appropriate.
  • Identify and utilise local assets, support networks, and community services to enhance each individual’s independence and quality of life.
  • Provide information, advice, and guidance to individuals and their support networks, enabling informed choices and greater control over the support they receive.
  • Work flexibly with individuals who have a broad range of needs, including supporting those whose behaviour may be challenging, using positive behaviour support principles.
  • Maintain accurate records, monitor progress against agreed outcomes, and contribute to reviews and service planning.

3 Month Contract With A local Authority

Job Purpose

As a SEND Progression Mentor within the Gateway Team, you will provide person‑centred, outcome‑focused support to young people and adults with Learning Disabilities, Autism, Sensory Needs, Physical Disabilities, or a combination of these needs. Working in the community, at the Southwark Resource Centre, and other appropriate locations, you will deliver strength‑based, time‑limited interventions that promote independence, wellbeing, social inclusion, and progression in line with each individual's Education, Health and Care Plan (EHCP). You will work flexibly to support individuals with varying levels of need, including those who may present behaviours of concern, using a positive behaviour support approach.
