Job summary

Wokingham Borough Council invites applications for a SEND Case Practitioner to join our Children’s Services SEND team on a full-time, permanent basis. You will manage a caseload of children and young people with SEND, acting as the key contact for families, schools and partner agencies.

You will coordinate Education, Health and Care (EHC) needs assessments and annual reviews, ensuring statutory processes are completed on time and that high-quality EHC Plans are developed with clear evidence and

Qualifications

• Experience working with children, young people and families with SEND.
• Strong understanding of SEND legislation, the SEND Code of Practice, and related processes.
• Excellent communication and relationship-building skills with families, schools and partners.
• Ability to analyse complex information and write high-quality EHC Plans.
• Highly organised with ability to manage priorities and maintain case records.

Responsibilities

• Manage a caseload of children and young people with SEND and act as key contact for families and partners.
• Coordinate Education, Health and Care needs assessments and annual reviews to meet statutory timescales.
• Gather and present information to support decision-making and development of EHC Plans.
• Work with education, health and social care partners to secure appropriate provision.
• Provide guidance to families and stakeholders in a person-centred approach.
