Job summary

Wokingham Borough Council is seeking a SEND Case Practitioner to join the Children’s Services SEND team on a full-time, permanent basis. You will manage cases, coordinate EHC needs assessments and work with families, schools and partners to secure appropriate provision.

The role requires experience with SEND, knowledge of relevant legislation, and excellent communication. The council offers generous annual leave, career development and a supportive environment.

Qualifications

• Experience working with children, young people and families with SEND.
• Strong knowledge of SEND legislation and statutory assessment processes.
• Excellent communication and relationship-building skills.
• Ability to analyse information and draft high-quality Education, Health and Care Plans.
• Highly organised with the ability to multi-task and meet deadlines.

Responsibilities

• Manage a caseload of children and young people with SEND as key contact.
• Coordinate EHC needs assessments and annual reviews within timescales.
• Gather, analyse and present information to support decision-making for EHC Plans.
• Work with education, health and social care partners to secure provision.
• Provide guidance to families and stakeholders with a person-centred approach.
