Tower Hamlets GP Care Group is recruiting a SEND Assistant Practitioner to join the 0-5 Health Visiting Service. You will support babies and children aged 0-5 with SEND or complex health needs and work with their families in homes, family hubs, nurseries, GP surgeries and clinics.

Under the supervision of the Specialist SEND Health Visitor, you will carry out developmental reviews, provide 1:1 and group support, and help families access the right services early.

Qualifications

• Experience of working with safeguarding children and reporting concerns.
• Experience supporting children and families with SEND or additional needs.
• Good written and verbal communication with people from diverse backgrounds.

Responsabilités

• Support delivery of the Healthy Child Programme (0-5) focusing on SEND and complex needs.
• Undertake delegated developmental reviews, assessments and screenings.
• Make referrals for speech and language, social communication, neurodevelopmental and other pathways.
