Job summary

Petersfield-based GP practice with three GP Partners, nurses and a supportive team seeks a motivated GP Partner. We value continuity of care and training, including fellowships, with a patient-centred approach and a collaborative environment.

The wider team includes three GP Partners, five nurses, HCA/phlebotomists, and a Wellbeing PCN. We offer 30 days holiday plus 8 bank holidays (pro rata for part-time staff), sick pay, and a supportive working culture.

Qualifications

• GP with clinical independence and the ability to work collaboratively within a multi-disciplinary team.
• Experience in delivering patient-centred care and continuity of care.

Responsibilities

• Make professional, autonomous decisions in relation to presenting problems.
• Assess health care needs of patients with undifferentiated or undiagnosed issues.
• Screen patients for disease risk factors and early illness signs.
• Develop care plans in consultation with patients following current protocols.
• Provide counselling and health education to patients.
• Admit or discharge patients and refer to other care providers as appropriate.
• Record clear, contemporaneous notes and collect data for audits.
• Prepare computer-generated acute and repeat prescriptions.
• Prescribe in line with formulary when clinically appropriate.
• Contribute to attainment of QoF and local/national initiatives.
• Complete insurance and other medical reports as required.
