Job summary

Lakeside Healthcare Group is seeking a Salaried GP to join Oundle Medical Practice, working with the East Northants Neighbourhood MDT. You will deliver proactive, patient-centred care for long-term conditions and support patients to improve health outcomes and quality of life.

The role requires GMC registration, licence to practise, NHS Performers List inclusion, and experience with boarding communities. Board-certified applicants with paediatric care experience are preferred.

Qualifications

• GMC registered and licence to practise.
• Experience with boarding communities; MOSA knowledge.
• Primary care experience with children/young adults preferred.

Responsibilities

• Delivering comprehensive Long-Term Condition (LTC) reviews and supporting achievement of QOF targets.
• Developing and maintaining high-quality, personalised care plans tailored to patients' individual needs and goals.
• Leading Future Care Planning conversations, helping patients understand and plan their ongoing healthcare needs.
• Facilitating Advance Care Planning discussions and ensuring patient wishes are documented.
• Supporting Respect Plans and promoting shared decision-making.
• Working collaboratively within a multidisciplinary team to provide coordinated and holistic care.
• Contributing to service development, quality improvement initiatives and innovative models of care.
