Job summary

Lakeside Healthcare’s Oundle Medical Practice in Northamptonshire invites a motivated Salaried GP to join our friendly primary care team. You will deliver patient-centred care focusing on long-term conditions, personalised care planning, advance care planning, and future care conversations.

Working with the East Northants Neighbourhood multidisciplinary team, you will lead LTC reviews, develop tailored care plans, support Respect and Advance Care Planning, and ensure patient goals and

Qualifications

Must be GMC registered and licensed to practise.
Be included on NHS Performers List.
Experience with boarding communities (MOSA) preferred.
Experience delivering primary healthcare to children/young adults.
Committed to high standards of patient care and clinical governance.

Responsibilities

Deliver LTC reviews and support achievement of QOF targets.
Develop personalised care plans tailored to patients' needs and goals.
Lead Future Care Planning conversations with patients.
Facilitate Advance Care Planning discussions and document preferences.
Support and complete Respect Plans and promote shared decision-making.
Work with a multidisciplinary team to provide coordinated care.
Contribute to service development and quality improvement initiatives.
