Due to our continued commitment in supporting people suffering with substance misuse problems, Change Grow Live are looking for a Homeless Recovery Coordinator to join our multidisciplinary team in Tower Hamlets RESET Treatment Service. The team supports service users with drug issues from point of entry into the service and through their treatment/recovery journey.

Our ideal candidate will have the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs.

Full Time Hours: 37.5 per week

Full time Salary Range: £27,861.26 - £32,002.35 dependent on experience (based on full time hours, pro rata for part time hours)

Allowance: £4,133.14 Inner London Weighting (pro rata)

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

Change Grow Live services and staff will provide wraparound support to rough sleepers and will work directly with emergency accommodation to support the staff with this group of people. We are looking for enthusiastic individuals and friendly faces to help support people in leading healthier lives.

About the Role:

  1. Establish clear and effective referral pathways for people experiencing homelessness, ensuring they can access the right health and care services.
  2. Build strong working relationships with local authorities, health and social care providers, and homelessness services to improve collaboration and outcomes.
  3. Attend relevant multi-disciplinary meetings to support links to primary care, mental health services, peer networks, community resources, and mutual aid groups.
  4. Liaise with all services involved in an individual's care to ensure smooth and secure transfers of support.
  5. Coordinate the transfer of individuals who are accommodated out of area, including referrals to new drug and alcohol services, liaison with pharmacies, and arranging bridging prescriptions where needed.
  6. Strengthen partnership working with housing services, voluntary organisations, and other agencies supporting rough sleepers.
  7. Ensure culturally appropriate support is available, particularly for hard-to-reach groups such as Eastern European and Sub-Saharan African communities.
  8. Use existing and new networks to broker support from strategic partners and community providers.
  9. Maintain continuity of care for individuals moving out of the area by ensuring planned and coordinated transitions.
  10. Hold cases for a minimum of 4–6 weeks following a service transition to provide stability and ensure successful engagement with new services.
About You:
  • You are committed to supporting people with lived experience of rough sleeping and substance misuse.
  • You bring experience of engaging individuals with complex and multiple needs and approach challenges with persistence and empathy.
  • You have knowledge of one or more of the following areas: homelessness, mental health, substance misuse, or the criminal justice system.
  • You take a trauma-informed, person-centred approach, recognising that flexibility and curiosity are key to building trust.
  • You are an attentive listener who adapts to individual needs and provides support in a compassionate and respectful manner.
  • You are culturally aware and committed to inclusive practices that reach diverse and marginalised communities.
  • You value collaborative working and are proactive in building strong partnerships across sectors to provide joined-up care.
