Join to apply for the Rough Sleepers Recovery Worker role at Change Grow Live

Due to our ongoing commitment to supporting individuals experiencing homelessness and substance misuse, Change Grow Live is seeking a Homeless Recovery Coordinator to join our multidisciplinary team in the Tower Hamlets RESET Treatment Service. The team supports service users from entry into the service through their treatment and recovery journey.

Key Responsibilities include:

1. Establishing referral pathways for homeless individuals to access health and care services.
2. Building partnerships with local authorities, health providers, and homelessness services.
3. Participating in multidisciplinary meetings to support links to various services.
4. Coordinating care transfers, including out-of-area placements and referrals to new drug and alcohol services.
5. Strengthening partnerships with housing, voluntary organizations, and community providers.
6. Ensuring culturally appropriate support for diverse and marginalized groups.
7. Using networks to broker support from strategic partners.
8. Ensuring continuity of care during transitions out of the area.
9. Providing case support for 4-6 weeks post-transition to ensure engagement and stability.

About You:

1. Commitment to supporting people with lived experience of rough sleeping and substance misuse.
2. Experience engaging individuals with complex needs with persistence and empathy.
3. Knowledge of homelessness, mental health, substance misuse, or criminal justice system.
4. Trauma-informed, person-centred approach with flexibility and curiosity.
5. Attentive listening skills and compassionate support.
6. Culturally aware and committed to inclusive practices.
7. Proactive in building partnerships for joined-up care.
