Job description

To participate in the professional provision of Pharmacy Services to thePrincess Alexandra Hospital NHS Trust and other organisations supported by thepharmacy department, while also being involved in specified project or groupwork.

Main duties of the job

As part of the role, you will be responsible for delivering a high standard of clinical pharmacy services, including prescription monitoring at ward level to ensure appropriate drug selection, treatment effectiveness, and the management of side effects. You will provide expert advice to prescribers and nurses, counsel patients on their medications, and ensure compliance with clinical care pathways, care bundles, and local guidelines. The role includes organising the timely supply of medicines, supporting formulary adherence, and participating in the medicine information service. You will also contribute to evidence-based pharmaceutical solutions by reviewing relevant literature and ensure all clinical activities are carried out in line.

Job responsibilities
  • To participate in clinical pharmacy services byscreening prescriptions, counselling patients, advising healthcareprofessionals, and contributing to safe, effective, and evidence-basedprescribing practices.
  • To supportdischarge planning and medication continuity by undertaking drug histories,transcribing discharge medication, liaising with GPs and community pharmacists,and facilitating early patient discharge through counselling and appropriatesupply.
  • To contributeto clinical governance and risk management through documentation of clinicalinterventions, participation in adverse drug reporting, attendance at wardrounds, and involvement in audit and therapeutic drug monitoring.
  • To dispense,distribute, and supervise the supply of medications, including controlled drugsand clinical trial items, in accordance with Standard Operating Procedures andensure accuracy and timeliness in prescription handling.
  • To manage theuse of pharmacy systems and automated dispensing technology, trackingprescriptions and supporting the accurate supply and documentation ofmedications across hospital wards and departments.
  • To ensurecompliance with statutory and local medicines regulations, including controlleddrug checks, formulary control, and storage and administration protocols withinclinical areas.
Person Specification
Qualifications
  • Degree in Pharmacy
  • GPhC Registration
  • Experience in Hospital Pharmacy
  • Experience of EPMA systems
  • Computer literate, knowledge of Microsoft office, patient information systems, and pharmacy dispensing/stock holding systems is required
