Job summary

East Kent Hospitals NHS Trust is seeking a Clinical Pharmacist for a rotational post across our three largest hospital sites. You will perform medicine reconciliations, screen drug charts and collaborate with multidisciplinary teams to ensure high-quality patient care.

The role includes facilitating safe discharges and supporting trainee pharmacists within an extensive in-house training programme. Applicants must be registered with the GPhC, possess strong written and oral communication skills,

Qualifications

• Registered with GPhC and able to practice as a pharmacist in the UK.
• Strong written and oral communication skills for multi-disciplinary teams.

Responsibilities

• Perform medicine reconciliations and screen drug charts.
• Work closely with multi-disciplinary teams to ensure optimal patient care.
• Facilitate safe patient discharges and ensure safe medicine provision on wards.
• Support the development of foundation pharmacists and trainees.
• Participate in rotational placements across main hospital sites.
