Zusammenfassung

Are you a compassionate Registered Nurse seeking a secondment to develop specialist respiratory skills within the Hospital @ Home Respiratory Team? This role blends acute and community nursing to support patients with acute and chronic respiratory conditions. Band 6 pay (£39,959–£48,117) for a 9-month fixed-term, full-time post.

Driving ability is required with mileage allowance provided. Assignment covers Taunton/Bridgwater or Yeovil/West Mendip areas, with 8-8 shift pattern and one in four

Qualifikationen

Registered Nursing degree is essential.
Registered with the Nursing Council (NMC) is essential.
Experience of managing respiratory patients within a community and acute setting is essential.
Understanding of working as part of a MDT is essential.
Enhanced understanding of Respiratory conditions is desirable.

Aufgaben

Deliver high-quality care to patients within the Somerset Respiratory service.
Provide autonomous practice within the community and acute settings.
Coordinate and evaluate complex care plans for patients with respiratory disease.
Work as part of a MDT to support early discharge and prevent admissions.
Support a 7-day service with weekend and unsocial hours as required.
Attend to patient needs and develop care pathways in collaboration with the team.
