Job summary

Hawthorn Medical Centre in Swindon seeks an experienced Nurse Practitioner to join our multidisciplinary primary care team. You will assess and manage patients with minor illness and deliver specialist respiratory services for asthma and COPD, within our Total Triage Hub.

The role emphasizes autonomous practice, high standards of care, and collaboration with GPs, ACPs, pharmacists, and community teams to improve access and outcomes.

Qualifications

• NMC registered nurse with UK practice eligibility.
• Advanced clinical assessment qualification or equivalent.
• Recognised qualification in Minor Illness management.
• Respiratory qualification or substantial experience in asthma/COPD.
• Master's degree in Advanced Clinical Practice.

Responsibilities

• Assess, diagnose, treat and manage patients with minor illness.
• Deliver specialist respiratory care and annual reviews for asthma/COPD.
• Perform spirometry where qualified and interpret results.
• Develop personalised care plans and optimise inhaler therapy.
• Collaborate with Total Triage Hub and multidisciplinary team.
