Job summary

Thames Reach Charity is seeking a Lead Worker for the Resettlement Support Team to assess clients, design move-on plans, and coordinate with partners across London to help people transition from street homelessness to stable housing.

You will document work, contribute to data collection, and build strong agency relationships while delivering a high standard of service. The role runs 37.5 hours weekly under a fixed-term contract until 31 July 2027.

Qualifications

• Strong commitment to high standards of service for clients, funders and partners.
• Experience working with vulnerable adults to access and maintain engagement with services.
• Excellent organisation and time management to handle many cases and meet deadlines.
• Ability to form strong working relationships with partner agencies and challenge them when needed.
• Proficient IT skills with Outlook, online records, Excel and Word on mobile devices.

Responsibilities

• Take the lead on assessing clients and devising sustainable move-on plans into suitable accommodation.
• Lead on building and sustaining relationships with participating partners in the pilot.
• Link clients to post-resettlement support to sustain tenancies.
• Lead and represent the team at multi-agency meetings.
• Accurately record all work within given timeframes.
• Lead within the team on data collection and reporting.
