Job summary

North Hampshire Urgent Care is seeking experienced registered Nurses or Paramedics to join our Remote Monitoring Service from our Farnborough Head Office. This fixed-term role supports digital pathways that identify deterioration early and reduce hospital admissions.

The service operates seven days a week 08:00–20:00, with flexible shifts across weekdays, weekends and bank holidays, and supervision of Clinical Support Workers in collaboration with GPs and multidisciplinary teams.

Qualifications

• Current and full membership with a professional registering body (NMC, HCPC).
• Educated to degree level.
• Evidence of post-registration training relevant to long-term conditions, urgent care or remote monitoring.

Responsibilities

• Lead and supervise non-registered Clinical Support Workers with clinical oversight.
• Oversee remote monitoring service including level 2 and level 3 pathways.
• Respond to digital platform alerts and escalate as needed.
• Collaborate with GPs, community teams and MDT colleagues.
• Maintain accurate patient records in EMIS Web.
