Job summary

Stroke Association invites applications for three Bank Support Coordinator roles. These 16-hour per week, home-based positions provide flexible support to stroke survivors and their families, reporting to a Bank Service Delivery Coach.

Qualifications

• Experience of providing person-centred support to vulnerable people or people with a disability or long-term health condition and their carers
• An effective listener with the ability to communicate clearly and effectively with a wide audience including anyone experiencing communication difficulties, primarily via telephone but also via written and digital methods such as video calls
• Experience of using technology and IT systems to record and maintain client data
• Emotionally resilient with the ability to handle a variety of calls, potentially dealing with complex and challenging situations whilst working in your own home

Responsibilities

• Work with stroke survivors and carers to identify their needs, providing person centred support which enables them to achieve their desired outcomes
• Deliver an effective service in line with our case management principles
• Effectively utilise a range of internal and external resources, in meeting the identified support needs of an individual
• Take responsibility for keeping yourself well, physically and mentally, seeking support of peers and your manager when complex or challenging calls test your resilience
• Support stroke survivors to make informed lifestyle changes which will help them to prevent further strokes
