Stroke Association is seeking a Bank Support Coordinator to provide flexible, home-based support to stroke survivors and their families. This casual, 16-hour-per-week role supports services during periods of limited capacity, with occasional travel for team meetings.

You will work under a Bank Service Delivery Coach and respond to unplanned absences across localities. The role focuses on person-centred support, case-management-aligned service delivery, and collaboration with internal teams to

Qualifications

• Experience of providing person-centred support to vulnerable people or those with disabilities.
• Excellent listening and clear communication to a wide audience, including via telephone and written/digital methods.

Responsabilités

• Work with stroke survivors and carers to identify needs and provide person-centred support to achieve outcomes.
• Deliver effective service in line with case management principles.
• Utilise a range of internal and external resources to meet identified needs.
• Maintain personal well-being and seek support when dealing with challenging calls.
• Support stroke survivors to make informed lifestyle changes to prevent further strokes.
