Job description
Overview

Our new Chorley ABI Service will provide bespoke adapted supported living services for a man moving out of a hospital setting. The service will offer an empowering model of support which will enable him to lead a fulfilling and valued life – being part of his family, participating in his local community, developing independent living skills and achieving his personal goals. We are looking for individuals who may share his interests – like keeping fit, video games, listening to music, puzzles, bowling, cinema, football, snooker and socialising.

Responsibilities
  • Work dynamically between Chorley and his current placement to ensure the smooth development of the service and excellent transitions.
  • Provide person-centred support to enable independent living, community participation, and involvement with his family.
  • Assist in helping the client achieve personal goals and develop independence skills.
