Job summary

North Northamptonshire Council is seeking Relief Community Enablement Workers to support adults with disabilities in their own homes. You will help customers live independently, provide personal care, assist with daily activities, manage medication and liaise with families and external agencies based on care plans.

Qualifications

  • Satisfactory clearance of an enhanced DBS check.
  • Educated to GCSE or Equivalent
  • Care Certificate (or willing to work towards)
  • Level 2 or above in Health and Social Care or equivalent (or willing to work towards)
  • Proven experience in health or care work
  • Experience of delivering care services in a community-based setting
  • Able to travel effectively to various locations, using a vehicle
  • Demonstrate an interest in working in this area and the ability to undertake the role
  • Good verbal and written communication skills. Sufficient to write clear, concise, and accurate reports and to enable effective communication with a number of agencies.
  • Excellent interpersonal skills with the ability to listen and interact effectively with and on behalf of customers and to react appropriately to a crisis situation
  • Practical approach to resolving problems and achieving results
  • Flexible, adaptable, and tolerant to meet the needs of the service
  • Able to meet the physical requirements of the role
  • Ability to attend, successfully complete and understand the need for mandatory training
  • Able to inform customers on benefits and application systems

Responsibilities

  • To support people within their own home to live an independent life.
  • With reference to the individual's care plan provide practical and emotional support to customers to help them maintain independent living.
  • Assist customers with physical disabilities to carry out activities, hygiene routines and manage medication as directed within their care plans to help maintain health and wellbeing.
  • Assist customers to undertake practical tasks to help maintain their physical wellbeing and/or improve their living conditions.
  • With reference to the individual's care plan promote independence through the provision of information to customers and through liaison with carers, internal and external agencies.
  • Part of your role will be to support people within their local community enabling them with volunteer placements, budgeting, social activities, and future planning of goals.
  • You will be responsible for updating daily record sheets, any communication with outside agencies and ensuring that any changes to the individual's needs, are documented and reported to ensure that the care plan is a live document, whilst utilising departmental electronic record systems.
  • Undertake risk assessments, manual handling, and medication assessments in order to safeguard customers and staff, as well as to meet service standards.
  • You will be regularly reviewing the customers progress, monitoring the effectiveness and efficiency of the support plan whilst documenting adjustments where necessary.
  • You will ensure that all changes within the persons care package are communicated back to managers.

Become a Community Enablement Worker!

We're seeking to recruit some great new Community Enablement Workers to join our Specialist Support for Younger Adults Team. We're looking for people with the right values, a positive attitude, and a passion for supporting others. Could this be you?

Our customers are aged 18-65 and have a range of physical disabilities, learning disabilities and mental health needs.

You'll help them live as independently as possible by providing support with:

  • Personal care
  • Manual handling
  • Medication
  • Food and drink support
  • Finances
  • Shopping and community access

We support a mix of customers with both longer-term and short-term needs. Some people require ongoing support, whilst others access our enablement service for a shorter period, which could be anything from a few weeks to several months. During this time, you'll work alongside them to build confidence, develop skills, regain independence and achieve personal goals that enable them to live as independently as possible.

Working Patterns Available

As a Relief Community Enablement Worker, you'll have the flexibility to work across a variety of shifts that meet the needs of the service. Our service is open Monday to Sunday, 365 Days a year.

Typical shift patterns are:

  • Day Shifts: approximately 7:00am to 3:00pm
  • Evening Shifts: approximately 3:00pm to 11:00pm

Purpose of the job

To enable them to remain independent whilst supporting their wellbeing, rights and choices. To also support with any reablement needs and providing personal support where required.
