Job summary

North Cheshire and Mersey NHS Foundation Trust in Warrington is seeking a Band 5 Registered Nurse to support patients in the Padgate House Intermediate Care Unit. You will conduct holistic assessments, manage rehabilitation, and help patients achieve safe discharge home while delivering compassionate, evidence-based care.

You will work within a friendly, multidisciplinary team, providing high-quality, person-centred care, and contributing to service improvement.

Qualifications

• Post-registration experience with nursing.
• Ability to communicate sensitive information to patients, relatives and carers.
• Verbal, written and digital technology skills.

Responsibilities

• Provide nursing assessment and develop/implement care plans.
• Deliver rehabilitation and support Independence and goals.
• Provide clinical supervision to staff and students.
• Assist with admissions, discharges and coordination of care.
