North Cheshire and Mersey NHS Foundation Trust is seeking a Band 5 Registered Nurse to join the Intermediate Care Unit at Padgate House. The role focuses on holistic nursing assessments, rehabilitation, and collaborative care planning to support patient independence and safe discharge.

You will work within a friendly multidisciplinary team, contribute to care planning, and promote evidence-based practice while developing professionally in a trusted NHS environment.

Qualifikationen

• Awareness and understanding of Intermediate Care Nursing and pathways of care.
• Audit and research.

Aufgaben

• Provide nursing assessment of patients; plan, develop and evaluate care for changing health needs.
• Provide rehabilitation for step down and step up patients in Intermediate Care.
• Provide clinical supervision to other staff including students.
• May be responsible for routine work allocation and checking tasks of support staff.
• Provide health promotion information and advice.
