Are you a compassionate and enthusiastic Registered Nurse looking for a rewarding career where you can make a real difference every day? Join our dedicated team in Padgate House Intermediate Care.
Working within our 35-bed unit, you will support patients to maximise their independence, achieve their rehabilitation goals, and prepare for a safe discharge home or to their usual place of residence.
This is an exciting opportunity to work in a friendly, supportive, and forward-thinking multidisciplinary team where your nursing skills will have a direct impact on patients' recovery and quality of life. No two days are the same, and you'll play a vital role in helping patients achieve their rehabilitation goals while delivering high-quality, person-centred care. We are looking for someone who is passionate about rehabilitation, enjoys working collaboratively, and is committed to providing compassionate, evidence-based care. In return, you'll benefit from a welcoming team, opportunities for professional development, and the satisfaction of seeing the positive difference your care makes every day. As a Band 5 Registered Nurse, you will provide safe, effective, and compassionate nursing care to patients within our Intermediate Care Unit, supporting them through their rehabilitation, recovery, and reablement journey. You will complete holistic assessments, develop and review personalised care plans, and work with patients to promote independence and achieve their individual goals.
You will deliver a high standard of clinical care, including medication administration, monitoring patient progress, recognising changes in clinical condition, and escalating concerns appropriately. You will support safe admissions and discharges, ensuring patients receive coordinated care throughout their stay. Working collaboratively within the multidisciplinary team, you will contribute to a positive recovery-focused environment, working closely with patients, families, carers, and colleagues to deliver person-centred care. You will advocate for patients, promote dignity and choice, and ensure their individual needs and preferences remain at the heart of everything we do.
You will contribute to maintaining high clinical standards through accurate documentation, effective communication, infection prevention practices, and participation in service improvement. You will also support learners and colleagues, while continuing to develop your own knowledge, skills, and professional practice.
Flexible working will be considered for all roles.
