Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting a Band 6 Clinical Charge Nurse for the Physical Rehabilitation Inpatient Ward. You will lead care planning, coordinate multidisciplinary input and advocate for patients with complex rehabilitation or end-of-life needs.

This role requires delivering high-quality clinical care without direct supervision, supporting the Ward Manager, and contributing to service improvement within a community hospital environment to support

Qualifications

• Registered nurse with current NMC registration.
• Experience in rehabilitation or end-of-life care is desirable.
• Ability to lead care planning and coordinate a multidisciplinary team.

Responsibilities

• Coordinate, deliver and maintain high standard day-to-day clinical care.
• Create and deliver comprehensive care plans for patients.
• Work with an integrated multidisciplinary team to support rehabilitation and timely discharge.
• Support ward development and provide mentorship to junior staff.
• Advocate for service users and uphold CARE values.
