Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting a Band 6 Clinical Charge Nurse for the Physical Rehabilitation Inpatient Ward. You will lead care planning, coordinate multidisciplinary input and advocate for patients with complex rehabilitation or end-of-life needs.

This role requires delivering high-quality clinical care without direct supervision, supporting the Ward Manager, and contributing to service improvement within a community hospital environment to support

• Registered nurse with current NMC registration.
• Experience in rehabilitation or end-of-life care is desirable.
• Ability to lead care planning and coordinate a multidisciplinary team.

Responsibilities

• Coordinate, deliver and maintain high standard day-to-day clinical care.
• Create and deliver comprehensive care plans for patients.

Job description

We are currently recruiting a Band 6 Clinical Charge Nurse for our Physical Rehabilitation Inpatient Ward. This is an exciting position for Registered Nurses who wish to take that next step in their career and who have an interest and passion for caring for patients with complex needs requiring a period of rehabilitation or end of life care. You will create/deliver care plans, contribute to Quality Improvement and will advocate for service users., To be responsible for co-ordinating, delivering and maintaining a high standard of day to day clinical care to all patients in the ward area.

To provide high standards of care for patients and their families by assessing, planning, implementing and evaluating care without direct supervision but within scope of professional practice.

To work as part of an integrated multi-disciplinary team visiting patients providing rehabilitation and health care in a community hospital environment to support their timely discharge from hospital.

Non clinically the individual will support other team members to develop their clinical knowledge. Participate and support with team development to ensure a quality service is provided.

To provide support to the Ward Manager (Band 7) and clinical leadership to the ward team and deputise for the Band 7 in their absence.
