You will help coordinate care at home, support flexible working, and engage with innovative community-based models to deliver safe, effective treatment while maintaining patient-centered care.

Qualifikationen

• Registered General Nurse / RMN / Practitioner or Allied Health Professional.
• Post-registration qualification or University Degree.
• Teaching and assessing qualification ENB 998 or equivalent.

Aufgaben

• Provide holistic leadership to the team and support the Clinical Team Lead.
• Lead and facilitate patient-focused, coordinated case management across primary/secondary care.
• Contribute to multi-disciplinary pathways to support self-management and early intervention.
• Work across caseload to identify patient needs and liaise with the appropriate services.
