Job summary

Harrogate and District NHS Foundation Trust in Harrogate is recruiting Band 5 Community Nurses to support care across inpatient, ED and community settings, based across four community bases and the new Hub at Harrogate Hospital.

You will deliver high-quality nursing care, plan and evaluate episodes of care, and work with a wide MDT. Induction and a Preceptorship programme are provided to support your development.

Qualifications

  • Registered nurse with NMC registration.
  • Ability to assess patients with complex health care needs and develop care plans.
  • Maintain accurate, timely and contemporaneous records.
  • Excellent verbal and written communication.

Responsibilities

  • Provide nursing care across unscheduled urgent community response, hospital at home and intermediate care.
  • Inreach into ED and acute frailty unit when required.
  • Plan, implement and evaluate episodes of care.
  • Plan and organise workload and delegate to other staff as necessary.

Job summary

Are you passionate about working in the complex area of frailty in the community? Are you a confident registered nurse and decision maker, who can think on their feet and embrace positive risk taking? Would you like to work in a dynamic, ground-breaking, and fully integrated intermediate care team? If the answer to the above is yes, then we would like to hear from you! Due to re-organisation and restructure in adult community services, we have brand new opportunities for band 5 registered nurses. Working to a home first agenda, the role is to support service delivery across inpatient, emergency department and community settings. Our service consists of Health and Social Care colleagues. Consultants, GPs, Advanced Care Practitioners, Urgent community response practitioners, Social Workers, Nursing, Occupational therapists, Physiotherapists, Discharge Team, rehabilitation assistants and Multi-Disciplinary Assistants all work together to support patient flow. We operate 7 days, 365 days a year to support some of our most vulnerable, older people with frailty. You will be based across our 4 community bases in addition to our new purpose-built Hub at Harrogate Hospital.

Main duties of the job
  • To undertake the provision of high standards of nursing care in relation to unscheduled urgent community response, hospital at home and intermediate care in the community.
  • When required inreach into acute services in ED and acute frailty unit and SDEC
  • To plan, implement and evaluate episodes of care.
  • To plan and organise workload and delegate to other staff when necessary.
  • To undertake nursing assessment, diagnosis and treatment appropriately.
  • To work alongside senior nurses and Matron to support the delivery of nursing intervention
  • To undertake intermediate care assessments on behalf of the team

Essential

Desirable
  • Teaching and assessment qualification/ Mentor qualification
  • Experience of Community working
  • Experience of risk management and governance
  • Commitment to clinical supervision
