Due to re-organisation and restructure in adult community services, we have brand new opportunities for band 5 registered nurses. Working to a home first agenda, the role is to support service delivery across inpatient, emergency department and community settings. Our service consists of Health and Social Care colleagues. Consultants, GPs, Advanced Care Practitioners, Urgent community response practitioners, Social Workers, Nursing, Occupational therapists, Physiotherapists, Discharge Team, rehabilitation assistants and Multi-Disciplinary Assistants all work together to support patient flow. We operate 7 days, 365 days a year to support some of our most vulnerable, older people with frailty.
