Job summary

Harrogate and District NHS Foundation Trust (HDFT) is seeking brand new Band 5 registered nurses to support a home-first intermediate care approach across inpatient, ED and community settings. You will work across four community bases and the new Hub at Harrogate Hospital, contributing to patient flow and high standards of nursing care.

As part of a dynamic integrated team, you will engage with a multidisciplinary network to assess, plan and deliver care for frailty in the community, including

Qualifications

• Confident registered nurse with decision-making ability.
• Band 5 level role in adult community services across inpatient, ED and community settings.
• Ability to plan, implement and evaluate episodes of care and manage workload.

Responsibilities

• Provide high standards of nursing care in unscheduled urgent community response, hospital at home and intermediate care in the community.
• When required, inreach into acute services in ED and acute frailty unit and SDEC.
• Plan, implement and evaluate episodes of care; plan workload and delegate to staff as needed.
• Undertake nursing assessment, diagnosis and treatment appropriately.
• Collaborate with senior nurses and Matron to support nursing interventions; undertake intermediate care assessments.

Due to re-organisation and restructure in adult community services, we have brand new opportunities for band 5 registered nurses.

Working to a home first agenda, the role is to support service delivery across inpatient, emergency department and community settings.

Our service consists of Health and Social Care colleagues. Consultants, GPs, Advanced Care Practitioners, Urgent community response practitioners, Social Workers, Nursing, Occupational therapists, Physiotherapists, Discharge Team, rehabilitation assistants and Multi-Disciplinary Assistants all work together to support patient flow. We operate 7 days, 365 days a year to support some of our most vulnerable, older people with frailty.

You will be based across our 4 community bases in addition to our new purpose-built Hub at Harrogate Hospital.
