Harrogate and District NHS Foundation Trust (HDFT) is seeking brand new Band 5 registered nurses to support a home-first intermediate care approach across inpatient, ED and community settings. You will work across four community bases and the new Hub at Harrogate Hospital, contributing to patient flow and high standards of nursing care.

Due to re-organisation and restructure in adult community services, we have brand new opportunities for band 5 registered nurses.
Working to a home first agenda, the role is to support service delivery across inpatient, emergency department and community settings.
Our service consists of Health and Social Care colleagues. Consultants, GPs, Advanced Care Practitioners, Urgent community response practitioners, Social Workers, Nursing, Occupational therapists, Physiotherapists, Discharge Team, rehabilitation assistants and Multi-Disciplinary Assistants all work together to support patient flow. We operate 7 days, 365 days a year to support some of our most vulnerable, older people with frailty.
You will be based across our 4 community bases in addition to our new purpose-built Hub at Harrogate Hospital.
