Job summary

Yorkshire Community Care in Leeds is seeking a skilled Staff Nurse (Complex Care) to deliver safe, person-centred nursing in clients' homes, managing complex needs and working with families and multidisciplinary teams.

You will provide clinical support including PEG feeding, catheter care, medication administration and wound care; maintain accurate records and participate in quality improvement activities to support service development.

Qualifications

  • Registered nurse with current NMC registration.
  • Minimum 2 years' post-registration clinical experience, preferably in community or complex care.
  • Proven competence in PEG feeding, catheter management, medication administration and respiratory support.
  • Excellent communication and interpersonal skills to liaise with families and partner agencies.
  • Flexible to work evenings or weekends as required.

Responsibilities

  • Deliver safe, person-centred nursing care in service users' homes, following care plans and best practice guidance.
  • Provide clinical support for complex needs including PEG feeding, catheter care, medication administration and wound management.
  • Administer and manage medications safely in line with policy and record keeping.
  • Monitor clinical condition, recognise deterioration and escalate as needed to the appropriate clinician or emergency services.
  • Maintain clear, accurate records using electronic and paper systems to ensure continuity of care and regulatory compliance.
  • Work collaboratively with families, carers and multidisciplinary teams to promote holistic care and positive outcomes.
  • Participate in clinical audits, incident reporting and quality improvement activities to support service development.
  • Contribute to training, mentoring and induction of less experienced staff where appropriate.
  • Adhere to safeguarding procedures and infection prevention and control policies.

Job Overview

We are looking for a skilled and compassionate Staff Nurse (Complex Care) to join Yorkshire Community Care's home care team. This role involves delivering high-quality, person-centred nursing care in people's homes, managing clinically complex needs and working collaboratively with families and external healthcare professionals.

Key Responsibilities
  • Deliver safe, effective and person-centred nursing care in service user' homes, following individual care plans and best practice guidance.

  • Provide clinical support for complex needs including PEG feeding, catheter care, medication administration, wound management and respiratory/airway support where required.

  • Administer and manage medications safely in line with policy, including controlled drugs procedures and accurate record keeping.

  • Monitor clinical condition, recognise deterioration and elevate concerns to the appropriate clinician or emergency services as required.

  • Maintain clear, accurate and timely clinical records using organisational systems to ensure continuity of care and regulatory compliance.

  • Work collaboratively with families, carers and multidisciplinary teams to promote holistic care and positive outcomes.

  • Participate in clinical audits, incident reporting and quality improvement activities to support service development and best practice.

  • Contribute to training, mentoring and the induction of less experienced staff where appropriate.

  • Adhere to safeguarding procedures, infection prevention and control policies and all statutory and organisational standards.

Person Specification
  • Registered nurse with current registration with the Nursing and Midwifery Council (NMC).

  • Minimum of 2 years' post-registration clinical experience, preferably with experience in community or complex care settings.

  • Proven clinical competence in complex care interventions such as PEG feeding, catheter management, medication administration and respiratory support; training for specific equipment can be provided.

  • Excellent communication and interpersonal skills with the ability to build positive relationships with people who use services, families and partner agencies.

  • Ability to assess risk, manage safeguarding concerns and respond calmly and professionally to incidents.

  • Organised and able to manage a caseload, prioritise tasks and work independently across a home care setting.

  • IT literate with experience of maintaining electronic and paper records to a high standard.

  • Committed to continuous professional development and reflective practice.

  • Flexible working approach to meet service needs, including occasional evening or weekend duties.
