Tynedale Hospice at Home (THH) has proudly supported the communities of Ponteland and West Northumberland for over 30 years. We are currently looking to recruit a Registered Nurse as a Care Coordinator supporting THH Nursing Service.
THH Registered Nurses are integral members of the multi-disciplinary team providing supportive and palliative care to a range of patients with life limiting illnesses. This post is primarily office based but includes undertaking clinical reviews and peer supervision / support when requiredensuring the delivery of evidenced based and person-centred care.

Main duties of the job
The post holder will be responsible for triaging inbound referrals and liaising with the patient and / or family / carer to provide high quality nursing care. THH works in partnership with the wider Primary Health Care Team and Specialist Palliative Care team to deliver clinical care to patients. Although the post holder has access to advice they are expected to work unsupervised. In addition to co-ordinating clinical care, the post holder is expected to supervise and appraise the work of others including support workers. The role also encompasses participating in the Traceability rota and assisting the Deputy Head of Care to ensure care provision meets the required standards.
Essential qualifications and skills include:
- First Level Registered Nurse with minimum three years post registration experience
- Experience of working without direct supervision in a health care environment.
- Excellent interpersonal skills.

Key Responsibilities:
Clinical Role
- Assessing, planning, implementing and evaluating care provision within their scope of practice
- Monitor and evaluate end of life care plans to measure progress and ensure the effectiveness of any interventions
- Maintaining concise and accurate records of care delivery within electronic patient record, including Caring for the Dying Patient documentation
- Maintain a duty of care to patients, carers and colleagues through the exercise of personal and professional accountability
Communication/Documentation
- Work with patients, their families and friends in a sensitive and flexible manner, demonstrating an awareness of the impact of death, dying and bereavement and recognising that their priorities and ability to communicate may vary over time
- Manage unpredictable and/or volatile situations effectively
- Facilitate communication with clients for whom English is not the first language and those who have communication/language difficulties
- Develop and maintain communication with people about difficult and complex matters or situations relation to End of Life Care
Assessment and Care Planning
- Understand the range of assessment tools and ways of gathering information, and their advantages and disadvantages
- Assess pain and other symptoms using assessment tools, pain history, appropriate physical examination and relevant investigations
- Undertake/contribute to multi-disciplinary holistic assessment and information sharing
Advance Care Planning
- Demonstrate an awareness and understanding of the legal status and implications of the Advance Care Planning process in accordance with the provisions of the Mental Capacity Act
- Show an understanding of Informed Consent and demonstrate the ability to give sufficient information in an appropriate manner
- Work sensitively with families and carers to support them as the patient decides upon their preferences and wishes during the Advance Care Planning process
Leadership, Supervision and Appraisal
- Promotes evidence-based practice through continued professional development and the sharing of best practice within the team and with others
- Participate in annual appraisal and training of Hospice Support Workers
- Promotes health and safety, undertaking risk assessments as necessary
- Participates in the writing of policies, protocols and procedures when required to do so
Overarching Values and Knowledge
- Have an awareness of the importance of contributing to the evaluation and change of services, participating as appropriate and involving patients in that process
- In the context of End of Life Care have an understanding and knowledge of:
o Their own professional role/boundaries and the part they play in patient care provision
o Legal and ethical issues
o Professional codes of conduct
o The impact of their own beliefs on practice
o Approaches to and theories of change, loss and bereavement

- To undertake mandatory training as required
- To undertake appropriate personal development to maintain and develop skills and knowledge as determined by annual appraisal. This is subject to the availability of resources and requirements of the Department.
