Archer Unit is a 20-bedded Physical Rehabilitation Unit providing care mainly for elderly patients with a wide range of medical and surgical conditions. Due to the complex nature of our patients' conditions, comprehensive discharge planning is essential. The role involves regular liaison with therapists, social workers, and the palliative care team when necessary, advocating for vulnerable patients with dementia and other cognitive impairments. Responsibilities include supporting and facilitating appropriate rehabilitation opportunities, ensuring safe discharges, and promoting patient independence. The position requires shift rotations across days, weekends, and nights.
