Job summary

At Rotherham Hospice, we believe in helping people live well to the very end. As part of our Hospice at Home team youll play a key role in delivering high-quality, person-centred, and compassionate care to people with complex and life-limiting conditions. Youll work within a multidisciplinary team, led by the Head of Hospice at Home, to provide holistic support to patients and those closest to them.

This role offers the opportunity to practice specialist palliative nursing in a supportive and forward-thinking organisation. You'll manage your own caseload, support junior colleagues, and ensure the consistent delivery of expert care in line with our strategic vision, Living Lifes Wishes. Main duties of the job

Clinical Care

  • Deliver high-quality palliative and end-of-life care, focusing on physical, psychological, emotional, and spiritual needs.
  • Undertake holistic assessments and develop personalised care plans in collaboration with patients, families, and the wider team.
  • Provide expert symptom management under the guidance of the Senior Nurse.
  • Administer medications safely and in accordance with legal and organisational policies.
  • Coordinate and prioritise your caseload, ensuring continuity and responsiveness of care.
  • Escalate complex clinical issues appropriately and contribute to clinical decision-making.
Team Support and Leadership
  • Support the supervision and development of junior colleagues, students, and volunteers.
  • Act as a positive role model and contribute to a compassionate team culture.
  • Deputise for Senior Nurses as required, including accepting referrals and supporting handovers.
  • Share knowledge and promote best practice within the team through mentorship and peer learning.
Patient Advocacy and Family Support
  • Build trusting, therapeutic relationships with patients and their families, supporting them through times of distress and uncertainty.
  • Act as an advocate for patients, ensuring their preferences and dignity are always respected.
  • Signpost families to additional support services as needed, offering information and reassurance.

We provide comprehensive, compassionate care through two primary patient services:

  • 24-hour, 14-bed Inpatient Service
  • 24-hour Hospice at Home Service
