An exciting opportunity has arisen for a Registered Nurse to join the Physical Health & Treatment Team within Stockport.
The successful post holder will hold the role of the Physical Health & Treatment Team Practitioner and will be part of a multidisciplinary team who administers anti-psychotic medication, offers the treatment of ECT and monitors the physical health of patients open to secondary mental health services.
The post holder will be based across multiple sites, promoting continuity of care and effective communication regarding the treatment and support for patients under the care of secondary care mental health services.

Take a lead in facilitating the day-to-day functioning of the Clozapine Clinic, with support and guidance from qualified professionals such as the nursing team, consultants and pharmacists.
Monitor the effects and side effects of Clozapine medication in the community, with guidance from relevant professionals, such as the nursing team, consultants and pharmacists.
To administer intramuscular injections and monitor the effects of these, with the knowledge of contraindications and side effects, being able to relay accurate information to patients in a format that they are able to understand in accordance with trust policies.
To undertake venepuncture in the context of physical health monitoring, after relevant training and ongoing guidance.
To undertake physical health checks, interpret results and respond to and report concerns appropriately. (No moving and handling required).
To take part in the running of the ECT clinic and perform duties as specified. Including ECT session handovers, management, and recovery of patients on the ECT pathway.
To be actively involved in the development of team-based nursing, assuming the responsibilities of assessing, planning, and implementing a programme of nursing care for individual clients within the team.
To support patients, plan and implement programmes of nursing care to meet their individual needs, referring to other agencies as appropriate.
To evaluate and modify nursing care in accordance with the Care Programme Approach and facilitate the development of discharge plans.
To actively participate in multi-disciplinary meetings and contribute to clinical decisions regarding patients in your care.
To take part in group caseloads, with the support of the team and management.
To keep accurate and timely professional records of nursing care, ensuring confidentiality is maintained in line with Trust policy.
To assist in the development of service standards and participate in clinical audit.
To participate in the development and implementation of new initiatives, incorporating researched based practices.
To participate in clinical supervision.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
